Vision coverage for eye exams, glasses, and contact lenses

You’re eligible to enroll in vision coverage if you’re a full-time employee, or a part-time employee working at least 30 hours per week.

Moneta offers the Delta Vision plan, which is part of the Eye Med Insight Network.

Understanding your vision plan

Exams, lenses, frames, and contacts are each covered once every calendar year.

There is no deductible to meet for the vision plan. You’ll pay a fixed-rate fee, called a copay, plus the amount over the plan allowance for frames and contacts. Below are key features of the plan.

Service You Pay
Exam $10
Materials$25
Lenses: Single, bifocal, or trifocal$25
Frames$0 copay; $150 allowance and 20% off the balance over the $150 allowance
Contacts (medically necessary)$25 copay; $250 allowance
Contacts (elective, conventional)$25 copay; $150 allowance and 15% off the balance over the $150 allowance
Contacts (elective, disposable)$25 copay; $150 allowance
Cost for coverage

You pay monthly premiums for vision coverage through payroll deductions. The amount you pay in premiums depends on which plans you choose and who you cover.
